Positive Ageing, Access and Inclusion

Council’s Positive Ageing, Accessibility and Inclusion Program supports older residents, people with a disability of all ages and vulnerable residents to improve their health and wellbeing, increase social connections and support their participation in community life.

Navigation Support Program

The navigation support program assists seniors and vulnerable people to access services and supports they need. This includes:

  • Assist residents in understanding the Aged Care system including My Aged Care (MAC), eligibility, access, assessment and how to find a service provider
  • Supporting residents to link in with other agencies including social services, health and community participation opportunities
  • Assist residents to become involved in active ageing programs
  • Arranging activities to support residents to be healthy, active, and connected
  • Linking Carers to support services and networks
  • Community advocacy
  • Assistance with access to emergency food relief
  • Implementation of Council’s Positive Ageing, Accessibility & Inclusion Plan 2025-2029
  • The Navigation Support Program is a free and confidential service.

We can come to your home or arrange for an appointment at a location that suits.

For residents aged 65 and over

Eligibility and access to all aged care services is determined by a Commonwealth assessment agency - either the Regional Assessment Services (RAS) or the Aged Care Assessment Service (ACAS).

The Commonwealth Home Support Program (CHSP) and Home Care Packages Program (HCP) help senior Australians access entry-level support services to live independently and safely at home.

CHSP and HCP services work with you to maintain your independence rather than doing things for you.  

Eligibility for these programs is based on your current level of family support, your needs and age.  

For residents under 65

You have a right to receive high quality care and services and to always be treated with respect and dignity. 

These rights and more are protected by the Charter of Aged Care Rights. The Charter provides the same rights to all consumers. These rights apply no matter what type of Australian Government funded aged care services you receive. 

It is a good idea for you and those involved in your care to familiarise yourself with your rights. All providers of Australian Government funded aged care must comply with the Charter.
